Why Ambulance Floor Securement Matters
Ambulance crashes are one of the leading causes of EMS provider injury and death. NIOSH research has consistently identified unsecured or improperly secured equipment — including stretchers, patients, and loose gear — as a primary injury mechanism in both crash and sudden-stop events.
Beyond the cot itself, the floor anchorage system is the structural foundation of everything in the patient compartment. L-track and floor anchor hardware must connect to the ambulance body's structural frame — not to the floor skin alone — to achieve the load ratings required by NFPA 1917 and KKK-A-1822F. The same L-track systems used in NEMT wheelchair vans are widely specified in ambulance patient compartments for exactly this reason: versatility, rated load capacity, and field-replaceable components.
🏥
Ground Ambulances
Type I, II, and III ground ambulances require compliant floor anchor systems for cot retention, supplemental patient restraint, and onboard equipment securement per NFPA 1917 and state EMS licensing requirements.
🚐
Inter-Facility Transport
Hospital-to-hospital and SNF transport vehicles carry both stretcher patients and wheelchair patients. L-track systems installed in these vehicles support both cot fastener integration and four-point wheelchair securement from the same floor hardware.
🏢
EMS Fleet Operators
Private EMS companies, fire department transport units, and hospital-based ambulance services all require certified floor anchorage, patient restraint belts, and accessory hardware — stocked and ready to ship from WheelchairStrap.com.
Applicable Compliance Standards
Understanding which standard applies to your vehicle and state is the first step to correct product selection. Here is a quick reference:
| Standard | Issued By | Covers | Applies To |
|---|---|---|---|
| NFPA 1917 | National Fire Protection Association | Design, performance & testing of new automotive ambulances. Cot retention, patient compartment, structural requirements. | New ground ambulances. Replacing KKK as the primary US standard. |
| KKK-A-1822F | US General Services Administration | Federal purchasing specification for ambulances. Cot retention must withstand 10G forward/rearward, 5G lateral, 5G vertical. | Vehicles built before NFPA 1917 adoption. Still referenced by many states. |
| CAAS GVS v3.0 | Commission on Accreditation of Ambulance Services | Ground Vehicle Standard for ambulances. Consensus-based alternative to NFPA 1917. Widely adopted for accreditation. | CAAS-accredited services and states requiring GVS compliance (2022+). |
| AMD 001–025 | Ambulance Manufacturers Division / NTEA | Manufacturing standards for ambulance bodies including static load testing of stretcher mounts and floor anchors. | Ambulance manufacturers and remount builders. |
| SAE J2249 | SAE International | WTORS hardware performance. Applies to floor anchor hardware and occupant restraint belts in the patient compartment. | Floor track hardware, occupant/patient restraint belts in all patient transport vehicles. |
| 21 CFR 880.6910 | FDA | Regulatory classification for wheeled stretchers and cot fastening systems as Class II medical devices. | Cot fastener systems. Note: Floor track hardware is not classified as a medical device. |
| State EMS Licensing | State EMS / DOH agencies | Each state has its own ambulance licensing standard referencing one or more of the above. Requirements vary by state and vehicle build date. | All licensed ambulance services. Verify your state's adopted standard before procurement. |

Products below are the same floor anchorage, restraint, and securement hardware used in NEMT wheelchair vans — selected for ambulance and inter-facility transport applications.
Cot Securement · SAE J3027 · Featured Product
Q'Straint AMBULOK — Universal Cot Securement System
The only SAE J3027 crash-certified cot securement system compatible with virtually any cot — no cot modification, no tools needed for install or removal. Built on Q'Straint's proven wheelchair docking technology and installable into any NEMT vehicle with OMNI Floor track.
Floor Anchorage
L-Track & Floor Anchor Systems
L-track (airline track) is the most widely specified floor anchorage system in ambulance patient compartments. It supports cot fastener integration, supplemental wheelchair securement, and equipment mounting from the same structural track — all from a single compliant floor installation.
Recommended for ambulance use:
• L-Track lengths — 36", 48", 60" sections
• L-Track mounting hardware and fastener sets
• L-Track end caps and filler strips
• L-Track accessories (connectors, extension plates)
• Slide 'N Click track (for compatible vehicles)
Patient / Occupant Restraints
Lap Belts, Shoulder Belts & Combo Assemblies
In inter-facility transport where patients are seated (wheelchair or captain's chair), occupant restraints are required at every seat position per ADA and FMVSS 208. In ambulances, supplemental patient restraint belts are used alongside stretcher restraint straps for bariatric patients, combative patients, or additional immobilization.
Recommended for ambulance / inter-facility use:
• Combo lap-and-shoulder belt assemblies
• Lap-only belts (for supplemental patient restraint)
• Shoulder belts and retractable shoulder assemblies
• Belt extensions (for bariatric patients)
• Postural / gait belts for seated patients
Wheelchair Securement
4-Point Tiedown Systems for Combo Vehicles
Inter-facility transport vehicles and some Type II ambulance conversions transport both stretcher patients and wheelchair users. Four-point tiedown retractors mounted to L-track provide compliant wheelchair securement from the same floor system used for cot fastener integration — no second installation required.
Recommended for combo vehicles:
• Q'Straint QRT retractors (L-Track)
• AMF Bruns ArcSystem retractors
• Sure-Lok SL Series L-Track retractors
• Electrical retractors for high-cycle operations
• Complete 4-point WTORS combo kits
Accessories
Belt Cutters, Webbing Loops & EMS Accessories
In an emergency, rapid patient egress may require cutting through restraint webbing. Every ambulance and transport vehicle should carry a belt cutter accessible to both crew positions. Webbing loops and securement accessories round out the patient compartment setup.
Recommended accessories:
• Belt / webbing cutters (EMS crew egress tool)
• Secure webbing loops (equipment tethering)
• L-Track accessories (joiners, extension plates)
• Mounting hardware and replacement fasteners
• Installation guides for all hardware
Complete Kits
All-in-One WTORS Kits for Combo Ambulance / Transport Vehicles
For inter-facility vehicles carrying both stretcher and wheelchair passengers, complete WTORS kits provide everything needed to equip one wheelchair securement position — tiedown straps, lap belt, shoulder belt, and hardware — in a single box, ready to install on existing L-track.
Kit options:
• L-Track WTORS combo kits (Q'Straint, AMF Bruns, Sure-Lok) • Slide 'N Click combo kits • Electrical retractor kits for solo operators
Q'Straint AMBULOK — Cot Securement System, In Depth
AMBULOK is Q'Straint's breakthrough universal cot securement system — purpose-built for NEMT vehicles and EMS transport, crash certified to SAE J3027, and designed to work with virtually every major cot brand already in the field.

Installs into any existing NEMT vehicle equipped with OMNI Floor track. Works with cots already in your fleet — bringing your entire operation into standard compliance fast, without replacing existing cots or rebuilding vehicles.
🖐️
Highly Intuitive
Operation is patterned around medic muscle memory — movements EMS and NEMT techs already know. Every component weighs 10 lbs or less. One-handed operation. Low-profile design. No tools needed for install or removal.
🔄
Effortless Flexibility
The low-profile AMBULOK docking system requires no tools for install or removal — allowing crews to quickly transition between cot and wheelchair transport configurations as needed on the same shift.
🚑
Universal Cot Compatibility
Compatible with Stryker Power PRO XT, Stryker Performance PRO XT, Ferno PowerFlex+ ST, Ferno ProFlexx, and most other cots in service — including those already in your fleet. The bracket installs on the cot with no permanent modification.
Common Use Case: Inter-Facility Transport Vehicles
Inter-facility transport is one of the fastest-growing segments of medical transport — and one of the most complex from a securement standpoint. These vehicles routinely carry both stretcher patients and wheelchair-using patients on the same run, requiring a floor system that handles both scenarios without re-conversion between trips.
✓ The Right Setup
Install L-track along the full length of the patient compartment. Use this track to mount your cot fastener system at the head-end position and four-point tiedown retractors at a forward wheelchair position. One floor system, two use cases, full compliance for both.
✓ Documentation for Both Patient Types
Wheelchair transport positions must document four-point securement per state NEMT Medicaid requirements. Stretcher transport requires cot retention documentation per NFPA 1917 / state EMS licensing. Our daily trip checklist covers both — download free here.
