AMBULOK Cot Securement: A Buyer's Guide for Ambulance Operators

By WheelchairStrap.com October 14, 2026 8 min read

For decades, ambulance cots were secured with proprietary "antler-style" floor fasteners — hardware locked to a single cot brand, expensive to replace fleet-wide, and increasingly out of step with modern crash standards. Q'Straint's AMBULOK system was built to close that gap: a universal cot docking system that brings existing cots into compliance without replacing them.

This guide walks through how AMBULOK works, which bracket and base system to pair with your cot, and what to check before you order.

What Is AMBULOK?

AMBULOK is Q'Straint's docking system for ambulance and NEMT cot transport. Instead of a cot-specific fastener, AMBULOK uses a bracket that installs on the cot without permanent modification, then docks into OMNI Floor L-track already installed in the vehicle — the same track system used for four-point wheelchair tie-downs. That shared floor infrastructure is what makes AMBULOK practical: a vehicle already outfitted for wheelchair transport can add cot capability, and vice versa.

The system is engineered around EMS "muscle memory" — one-handed, tool-free engagement and release, with individual parts weighing 10 lbs. or less. Crews can transition a vehicle between cot and wheelchair configuration in minutes.

Why SAE J3027 Matters

SAE J3027, published in 2014, is the crash standard that reshaped ambulance cot securement. Testing that informed the standard found that older antler-rail cot fasteners allowed roughly 30 inches of forward cot movement in a simulated 30 mph frontal impact — enough force to break the cot free and send it into the patient compartment. SAE J3027 requires that forward excursion be limited to 14 inches, effectively phasing out antler-style systems industry-wide.

AMBULOK is engineered to meet SAE J3027 using securement technology adapted from Q'Straint's wheelchair tie-down systems. For a deeper look at how SAE J3027 relates to the broader CAAS GVS and NFPA 1917 ambulance standards, see our upcoming guide on ambulance cot securement standards.

The Four AMBULOK Components

An AMBULOK installation has two parts: a bracket that mounts to the cot, and a base system that installs in the vehicle floor. You need one of each.

Component SKU What It Does
Manual Bracket Q04BR202 Cot-side bracket for manual (non-powered) cots — installs without cot modification
Power Bracket Q04BR201 Cot-side bracket for powered cots — installs without cot modification
Mechanical System Q10-018 Vehicle-side floor base with a mechanical (non-electric) locking latch
Electrical System Q10-021 Vehicle-side floor base that integrates with the vehicle's 12-volt system for push-button lock and release

Bracket and base are sold separately. A bracket alone will not secure a cot — it needs a compatible floor base (mechanical or electrical), and the vehicle needs OMNI Floor L-track already installed. Choosing mechanical vs. electrical is a matter of preference and budget, not cot compatibility — either base works with either bracket.

Cot Compatibility

AMBULOK is built to work with cots already in service, not just new purchases. Confirmed compatible models include:

  • Stryker Power-PRO XT (powered cot)
  • Stryker Performance-PRO XT (manual cot)
  • Ferno PowerFlex+ ST
  • Ferno ProFlexx

If your fleet runs a different cot model, contact our specialists before ordering — we'd rather confirm fit than have you guess.

Buying Considerations

  • Match the bracket to the cot type — powered cots need the Power Bracket (Q04BR201); manual cots need the Manual Bracket (Q04BR202)
  • OMNI Floor L-track is a prerequisite — if your vehicle doesn't already have it installed, that's an additional project before AMBULOK can go in
  • Electrical vs. mechanical is a workflow choice — electrical offers push-button convenience; mechanical avoids any wiring into the vehicle's electrical system
  • Dual-use vehicles benefit most — a vehicle that alternates between cot transport and wheelchair transport gets full value from the tool-free, minutes-long configuration swap

Plan for lead time. AMBULOK components can carry lead times of up to 90 days depending on configuration and current demand. Order well ahead of a planned installation date or fleet rollout — this is not a same-week fulfillment item.

Installation & Documentation

Each AMBULOK component ships with a manufacturer installation manual, and brochure/comparison materials are available to help you spec a fleet-wide order. Installation should be performed by a qualified technician familiar with OMNI Floor L-track systems — this isn't a bolt-it-yourself product given its role in crash protection.

Frequently Asked Questions

Common questions about AMBULOK cot securement.

Do I need to replace my existing cot to use AMBULOK?
No. AMBULOK is designed to bring cots already in service into compliance. The bracket installs on the cot without permanent modification, so most fleets can add AMBULOK to existing compatible cots rather than purchasing new ones.
Can I use AMBULOK without OMNI Floor L-track already installed?
No — AMBULOK docks into OMNI Floor L-track, so the vehicle needs that track installed first. If your vehicle doesn't have it, installing L-track is a separate project that needs to happen before AMBULOK components can go in.
What's the difference between the mechanical and electrical base system?
The Mechanical System (Q10-018) uses a manual locking latch with no wiring required. The Electrical System (Q10-021) integrates with the vehicle's 12-volt system for push-button lock and release. Both work with either bracket — the choice comes down to budget and workflow preference, not cot compatibility.
How long does AMBULOK take to arrive?
Lead times can run up to 90 days depending on configuration and current demand. Order well ahead of your planned installation date, especially for a fleet-wide rollout.
Is AMBULOK crash-tested?
AMBULOK is engineered to meet SAE J3027, the crash standard for ambulance-based cot securement equipment, using technology adapted from Q'Straint's wheelchair securement systems.
What if my cot isn't on the confirmed compatibility list?
Contact our specialists before ordering. We list only confirmed compatible cot models rather than assuming fit — call 800.884.6456 or email sales@wheelchairstrap.com with your cot make and model and we'll confirm compatibility.
